When submitting a Claim Form, you must include supporting documents to provide the necessary information for the Administrator to process your Claim. The documents required depend on:

  • The Class you are applying under
  • Whether you are submitting a Claim for yourself or on behalf of someone else as a representative

Providing the correct and complete documents ensures that your Claim is processed as efficiently as possible.

If you are eligible under one of these Classes, you can submit your Claim along with the required supporting documents.

The Claims Periods for the remaining Classes are not yet open. These will open at a later date.

What supporting documents do I need?

If you are submitting a Claim under the Removed Child Class or Removed Child Family Class, you will need to provide the supporting documents based on your specific situation. Representatives must meet all requirements of the Claimant and for you, the representative.

Additional requirements for representatives

If you are submitting a Claim on behalf of someone else, you must provide:

  • All required documents for the Claimant
  • Additional supporting documentation confirming your authority to act as a representative

Important: submit copies only

When submitting your Claim, only send copies of your identification (photocopy, scan, or photo). Do not send original documents.

To avoid delays, ensure:

  • Copies are clear and readable (not blurry, too light or too dark)
  • You provide images of both sides of your identification
