When submitting a Claim Form, you must include supporting documents to provide the necessary information for the Administrator to process your Claim. The documents required depend on:
- The Class you are applying under
- Whether you are submitting a Claim for yourself or on behalf of someone else (as a representative)
Providing the correct and complete documents ensures that your Claim is processed as efficiently as possible.
